Several prediabetes interventions exist based on evidence from the landmark Diabetes Prevention Program (DPP) study. The DPP study reported that moderate weight loss (5-7% of body weight, or ~10-15 lbs. for someone weighing 200 lbs.), counseling, and education on healthy eating and behavior reduced the risk of developing type 2 diabetes by 58%. Data presented at the ADA 2014 conference showed that after 15 years of follow-up of the DPP study groups, the results were still encouraging: 27% of those in the original lifestyle group had a significant reduction in type 2 diabetes progression compared to the control group.
"Traditionally, we transplant islets in the liver of the animal and never do it under the skin, in large part because the skin doesn't have the blood flow that the liver has for transporting insulin released by islets. And there are a lot of immune cells in the skin, so chances of rejection are high," said Raghu Mirmira, professor of pediatrics and medicine and director of the Diabetes Research Center at the Indiana University School of Medicine.

This 2013 paper http://www.ncbi.nlm.nih.gov/pmc/... on page 5 reported that after the 8 weeks on that 600 kcal diet 10 out of the 11 participants, so not all, of the Counterpoint study, as the study is now known, regained normal glucose metabolism, 3 months after resuming a normal diet 4 out of the 10 still had a normal glucose metabolism, 3 had an impaired glucose tolerance, 3 had better controlled diabetes, no more recent figures published in spite of the first publication had been published in Octobre 2011, which doesn't bode well for the long term outcome I'd say, I'd have expected them would to have reported the longer term results by now were they positive.
Data from the Swedish randomized study of gastric banding showed that a loss of 20% body weight was associated with long-term remission in 73% of a bariatric surgery group, with weight change itself being the principal determinant of glucose control (13). Dietary weight loss of 15 kg allowed for reversal of diabetes in a small group of individuals recently receiving a diagnosis (21). In individuals strongly motivated to regain normal health, substantial weight loss is entirely possible by decreasing food consumption (88). This information should be made available to all people with type 2 diabetes, even though with present methods of changing eating habits, it is unlikely that weight loss can be achieved in those not strongly motivated to escape from diabetes. Some genetic predictors, especially the Ala12 allele at PPARG, of successful long-term weight loss have been identified (89), and use of such markers could guide future therapy. It must be noted that involuntary food shortage, such as a result of war, results in a sharp fall in type 2 diabetes prevalence (90,91).
Diabetes is a costly disease, placing a high financial burden on the patient and the healthcare system. If poorly managed or left untreated, it can cause blindness, loss of kidney function, and conditions that require the amputation of digits or limbs. The role of physical activity must be considered. Increased levels of daily activity bring about decreases in liver fat stores (43), and a single bout of exercise substantially decreases both de novo lipogenesis (39) and plasma VLDL (92). Several studies demonstrated that calorie control combined with exercise is much more successful than calorie restriction alone (93). However, exercise programs alone produce no weight loss for overweight middle-aged people (94). The necessary initial major loss of body weight demands a substantial reduction in energy intake. After weight loss, steady weight is most effectively achieved by a combination of dietary restriction and physical activity. Both aerobic and resistance exercise are effective (95). The critical factor is sustainability.
Other studies have found that people with pre-diabetes or type 2 diabetes can go into remission through changes to their dietary and exercise habits. People who manage to achieve this with food alone will often express their excitement publicly by claiming they “cured” their diabetes with their diet. Pancreatic islet transplantation is an experimental treatment for poorly controlled type 1 diabetes. Pancreatic islets are clusters of cells in the pancreas that make the hormone insulin. In type 1 diabetes, the body’s immune system attacks these cells. A pancreatic islet transplant replaces destroyed islets with new ones that make and release insulin. This procedure takes islets from the pancreas of an organ donor and transfers them to a person with type 1 diabetes. Cinnamonium cassia and its relative C. burmanii are the types of cinnamon that have the best effect on diabetes symptoms. There have been numerous studies on cinnamon and, overall, they have shown cinnamon can slow stomach emptying and lower postprandial glucose levels. It also reduces glucose levels in Type 2 Diabetes Mellitus patients who have had poor diabetic control. It may also be helpful in lowering insulin levels, blood pressure, and A1C, and reduce AGE formation. This is a safe herb for diabetics. Treatment plans are designed around the pattern of insulin normally supplied by the pancreas throughout the day in someone without diabetes. The diabetic patients' fasting blood sugar levels dropped about 9% more when they took ginseng compared with when they took the placebo; glycosylated hemoglobin levels between the two groups differed by 4%, with the ginseng group being lower.

Initial clinical trial results, published in a 2012 PLOS One paper, reported that two doses of BCG spaced four weeks apart led to reductions in autoreactive T cells, an increase in Tregs and what turned out to be a transient increase in insulin production. In another study, albeit including only 30 people, those who were recently diagnosed and went on a very low-calorie diet for eight weeks experienced remission. That remission continued more than six months after their low-calorie diet ended. In people who have had type 2 diabetes for a long time, unfortunately, weight loss has a much more limited impact.
